This service for people with CAD is provided at no cost to you. It works with your PCP’s treatment plan and will:

• Help you better understand what heart disease is and what risk factors you need to know about.
• Keep you up to date on the latest changes in the treatment and prevention of heart disease.
• Help you learn about your risks and how to take action now to control them so that you stay as healthy as possible.
• Answer any questions you have about heart disease.
• Provide 24-hour nurse support to help you when you need it.
