The ACCESS Plus Chronic Obstructive Pulmonary Disease (COPD) Management Program

The goal of the ACCESS Plus COPD Management Program is to give you the information you need to understand COPD and help you better control symptoms when they occur.

Did you know that COPD is one of the most common chronic medical conditions in the United States? It affects millions of people and is the reason for millions of emergency department visits and hospitalizations. COPD causes many thousands of deaths each year. The good news is that you can learn how to manage your COPD so that you are in better control of your symptoms.

This service for people with COPD is provided at no cost to you. It works with your PCP’s treatment plan and will:

  • Help you better understand what COPD is and what risk factors you can do to manage or control your COPD symptoms.
  • Keep you up to date on the latest changes in management and treatment of COPD symptoms.
  • Help you better understand how taking steps to control your COPD starting now can have a positive effect on your health.
  • Answer any questions you have about COPD.
  • Provide 24-hour nurse support to help you when you need it.
