The ACCESS Plus Diabetes Management Program

The ACCESS Plus Diabetes Management Program gives people with diabetes the information you need to help you manage your diabetes and help to prevent or manage any problems or complications. Do you want to lower your chances of having other problems from your diabetes, such as heart disease, stroke, eye disease, kidney disease and nerve damage? Do you want to feel better, have more energy and stay healthy?

This service for people with diabetes is provided at no cost to you. It works with your PCP’s treatment plan and will:

• Help you better understand what diabetes is and what you can do to prevent, slow down, or manage diabetes problems
• Keep you up to date on the latest changes in prevention and treatment of diabetes
• Help you better understand how controlling your diabetes now can have a positive affect on your health
• Answer any questions you have about diabetes Provide 24-hour nurse support to help you when you need it
