The ACCESS Plus Heart Management Program provides people with heart failure with the information you need to help you feel your best and better manage your symptoms

Did you know that heart failure is on the rise among Americans? One third of newly diagnosed patients will be hospitalized within the first year of being diagnosed. Identifying the actions you can take to control your heart failure is an important part in managing your health!

This service for people with heart failure is provided at no cost to you. It works with your PCP’s treatment plan and will:

• Help you better understand what heart failure is and how you can manage your symptoms
• Keep you up to date on the latest changes in management and treatment of heart failure
• Help you better understand how managing your heart failure now can have a positive affect on your health
• Answer any questions you have about heart failure Provide 24-hour nurse support to help you when you need it
