Pay For Performance

Welcome to the expanded ACCESS Plus Pay For Performance Program. You felt that we should expand the program to include more clinical measures that reflect the work you do, and to reduce the administrative work to receive compensation. We heard you on both accounts.

Clinically, we have added measures such as lab tests completed, medications prescribed and referrals to specialty care. Examples include Fasting LDL testing for diabetics, ACE or ARB prescribing for CHF patients, or ophthalmology referrals for diabetics.

Many of the new items are claims-based: specialist, lab or pharmacy bills alert us to what has been done. However, we still need some clinical information from you. We can only get blood pressure readings from you, for example. Claims from a laboratory also don’t include the lab result. You are the critical link to outstanding patient care, as well as successful compensation from the ACCESS Plus Pay For Performance Program.

Please take a look at the documents linked below for more information, especially the Summary of P4P Payment Opportunities. You will see that our program provides up to $300 per diabetic patient per year to practices that provide excellent clinical care.

We hope these additional dollars support practices as they move towards the Advanced Medical Home Model as proposed by the American College of Physicians, The American Academy of Family Physicians and the American Academy of Pediatrics.
