Pennsylvania Medicaid Adult HIV Clinical Practice Guidelines
The guideline was developed to help practitioners care for adult Medical Assistance recipients with HIV/AIDS. It was created using currently existing national protocols and is intended to be broad enough to cover all adult members living with HIV/AIDS and allow for accepted variations in clinical treatment and pharmaceutical utilization.

Health History Forms
We are pleased to announce the availability of initial pediatric, adolescent and adult history forms. The forms are designed to be completed by the patient prior to being seen by the PCP. The forms will assist in obtaining a comprehensive medical, social and family history for ACCESS Plus patients of all ages. The appropriate form can be completed at the initial visit and reviewed yearly by the PCP. The forms are easily reproducible or they may be utilized as templates for the electronic record.
New Age-specific Well Child / EPSDT Documentation Forms
We are proud to announce the availability of the new age-specific Well Child / EPSDT documentation forms on the website. These forms were prepared to assist in providing comprehensive EPSDT documentation for ACCESS Plus enrollees from birth to 20 years. The forms are easily reproducible and may be utilized as templates for electronic medical records.
Located on the second page of each exam form are the standard CPT, Diagnosis, and Referral codes related to EPSDT that may assist your billing personnel. Remember, complete screens are reimbursement at the enhanced EPSDT rate.
These forms can be used for both Well Child and EPSDT visits.
