ACCESS Plus is Pennsylvania’s Enhanced Primary Care Case Management Program (EPCCM). ACCESS Plus, administered under the Department of Public Welfare (DPW), is operational in the 42 counties where HealthChoices is not mandatory. Most Medical Assistance (MA) Enrollees who reside in the 42-county service area and who are not enrolled in a voluntary managed care organization will receive health care through ACCESS Plus.
The ACCESS Plus program enables Enrollees to choose their own Primary Care Providers (PCPs), while receiving the advantages of active care coordination, and if eligible case management, and, disease management services.
ACCESS Plus emphasizes health education and stresses the importance of preventative treatment. The goals of ACCESS Plus are:
- To improve access to primary care and provide a medical home
- To improve access to health care services for MA Enrollees
- To improve the quality of health care available to MA Enrollees
- To provide access to care management
- To stabilize Pennsylvania’s MA spending
- To support the principles and strategies of the Prescription for Pennsylvania initiative
DPW will reimburse ACCESS Plus PCPs on a Fee-for-Service (FFS) basis and offer enhanced FFS rates for selected primary care services provided by PCPs.
DPW has selected APS Healthcare to operate ACCESS Plus, with each organization working collaboratively to provide the two major components of ACCESS Plus – EPCCM and DM.
