The ACCESS Plus program uses a non-invasive approach to working with providers and their office staff.  Providers’ time and clinical expertise are valuable, and ACCESS Plus will pay providers when they work collaboratively with the program to improve patient health.

The ACCESS Plus Pay for Performance (P4P) Program is to encourage providers and office staff to:

• Actively participate in the ACCESS Plus Disease Management Program
• Deliver measurably high quality care that is consistent with evidence-based national guidelines
• Demonstrate improvements in the quality of care and services delivered to enrolled beneficiaries over time

Qualified providers who sign up to participate in the P4P Program receive supplemental payments for the following:

• Assisting program staff in enrolling DM program eligible patients by contacting program eligible patients to encourage them to participate in the program
• Becoming a registered smoking Cessation Counselor
• Accepting new ACCESS Plus members
• Developing a Pediatric Care Plan
