Advance Directives – Also known as living wills, are instructions given by individuals specifying what actions should be taken for their health in the event that they are no longer able to make decisions due to illness or incapacity. If you would like more information on Advance Directives, talk to your PCP or call the ACCESS Plus Helpline at 1-800-543-7633. You can also click here to download a booklet “Understanding Advance Directives for Health Care” which includes forms to complete.
American Academy of Pediatrics - Information on Advocacy, Research, Membership, You and Your Family, Professional Education, and Publications. The You and Your Family link gives detail on immunization schedules, safety and injury prevention, and sports issues.
American Dental Association - Information is available for both Dental Professionals and the Public. Professionals can access information on government and advocacy, education and careers, and meetings and events. The public can access oral health topics and the Find a Dentist directory and search for ADA Seal products.
BrightFutures.org - A national initiative with the mission to promote and improve the health and well-being of infants, children, and adolescents.
Center for Disease Control and Prevention - The CDC is one of the 13 major operating components of the Department of Health and Human Services (HHS), which is the principal agency in the United States government for protecting the health and safety of all Americans and for providing essential human services, especially for those people who are least able to help themselves. The Center for Disease Control and Prevention offers information on health topics. Data and statistics on many diseases, disorders, and occurrences are also available.

Department of Health - The PDH website offers information on Programs for Men, Women, and Children as well as information on Health Planning and Assessment and Health Promotion and Disease Prevention.

DPW - Other Services - The Department of Public Welfare offers a variety of services. National SAFE KIDS Campaign - Here you can find a numerous amount of safety tips. You can also learn how to get involved and see recent recall information. The Teacher's Desk and Kid's Corner offers fun games and puzzles that also teach kids about safety.
Nutrition in the Kitchen (2nd Edition) from the Healthy Weight Program - Designed with the whole family in mind, Nutrition in the Kitchen allows you to search for a specific recipe or browse categories such as side dishes or snacks. The Statewide Immunization Information System (SIIS) is a population-based immunization tracking system that is being developed to aid in achieving complete and timely immunizations for all people, particularly the age group most at risk, birth through two years. The registry is now being implemented in the public sector; future efforts will focus on implementation in the private sector. PA VFC quality assurance site surveyors are currently identifying providers who are interested in piloting the registry.
The Pennsylvania Association of Community Health Centers - The state primary care association representing Federally Qualified Health Centers and other like-mission providers located in underserved areas throughout the Commonwealth. PACHC programs and services are designed to strengthen, promote and grow health centers in today's complex and constantly changing health care environment.
